Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license without disciplinary actions or practice limitations.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Perform comprehensive assessments for high-risk plan members in home, nursing home, and senior housing settings.
• Conduct medication reviews and point-of-care testing as needed.
• Engage in advanced care planning discussions with patients and families.
• Collaborate with an interdisciplinary care team to support rehabilitation and ongoing needs.
