Job summary

Optum Home & Community Care in Norfolk, VA, is seeking a Per Diem Nurse Practitioner for the Senior Community Care program to serve high-risk Medicare members across home, assisted living, and long-term care settings. You will perform comprehensive assessments, medication reviews and advance care planning in a patient-centered model.

The role emphasizes collaboration with an interdisciplinary team, care coordination, and flexible afternoon/weekend coverage with an anticipated ~24 hours per month

Qualifications

Active ANCC or AANP Family/Adult/Geriatric certification.
Active, unrestricted NP license and ability to obtain before start date.
Active DEA license/prescriptive authority or ability to obtain post-hire.
Active driver's license and reliable transportation.

Responsibilities

Perform comprehensive assessments, medication reviews, and advanced care planning discussions.
Provide care in home, nursing home, assisted living, and senior housing environments.
Collaborate with an interdisciplinary care team to support complex patient needs and transitions.
Support per diem schedule with focus on afternoons and weekends.
