Qualifications

• Active NP certification (ANCC or AANP) in Family/Adult/Geriatric Primary Care.
• Active, unrestricted NP license without disciplinary actions.
• Active DEA license or ability to obtain after hire and establish collaborative practice if required.

Responsibilities

• Conduct comprehensive assessments and medication reviews.
• Perform point-of-care testing and advanced care planning discussions.
• Coordinate with an interdisciplinary team to support rehabilitation and ongoing care needs.
• Provide care across home, nursing home, assisted living, and senior housing environments.
