Job summary
Optum in Salem, OR is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team. This role emphasizes home-based care for seniors, with a focus on quality outcomes and a manageable workload.
The position offers a 24 hours per month commitment, flexible scheduling, and no on-call requirements. The ideal candidate will perform comprehensive assessments, coordinate care with interdisciplinary teams, and support advanced illness planning while delivering
Qualifications
- Graduate of an accredited advanced practice program (NP/PA).
- Active state license and certification to practice in Home Care.
- DEA license or prescriptive authority and ability to obtain if needed.
- Experience in senior living, geriatric or home care is preferred.
Responsibilities
- Perform comprehensive assessments of patients in home/senior living settings.
- Manage acute and chronic conditions with appropriate diagnostics and interventions.
- Document clearly and accurately per CMS guidelines.
- Collaborate with interdisciplinary care teams and care facility staff.
- Engage in advanced illness and care planning conversations.
