Job summary

Circadia Health in St. Joseph, MI seeks a Nurse Practitioner to advance AI-enabled care across skilled nursing facilities and home health environments.

You will see patients first, act on predictive signals in real time, and coach facility teams to drive proactive interventions that reduce readmissions. In SNFs you’ll round on new admits and flagged patients while extending the same proactive model into patients’ homes.

Qualifications

• MI NP license with ANCC or equivalent in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English required

Responsibilities

• Direct patient care across SNF and home health; assess and plan care.
• Train staff to integrate Circadia into clinical workflows at point of care.
• Interpret trend-based insights; prioritize high-risk patients for interventions.
• Complete documentation on the same day.
• Perform high-volume rounding (25+ visits/day) across designated sites.
• Provide mentorship and collaborate with clinical ops/product teams.
