Responsibilities

• Link patients with aftercare services prior to discharge and aid access to outpatient services.
• Evaluate past successes and barriers to avoid re-hospitalization.
• Provide post-evaluation follow up face to face or phone support as clinically indicated.
• Monitor patients onsite during disposition planning or boarding.
• Respond to emergency phone calls with triage and resources.
