Job summary

High Point & Affiliated Organizations, based in Southeastern Massachusetts and serving Brockton, is seeking a Clinician to provide direct care to patients, including assessments and counseling, while maintaining treatment plans and daily notes. The role requires affiliation with behavioral health and substance use treatment services.

The position involves discharge planning, aftercare coordination, and collaboration with treatment teams to support patient recovery.

Qualifications

• Bachelor's degree in behavioral health or related field required.
• Knowledge of relationship between substance abuse and mental health issues and/or self-help programs preferred.
• Experience with diagnostic, assessment and case management processes.
• Ability to promote knowledge about psychiatric illness and addiction symptoms.

Responsibilities

• Maintain and submit licensure or certificates necessary for the position.
• Comply with Federal Regulations, 42 CFR Part 2, confidentiality policies and HIPAA.
• Participate in Treatment Team Meetings and maintain assigned caseload.
• Provide direct patient care including assessments, individual counseling, discharge planning and treatment plans.
• Maintain medical records and ensure timely entries.
• Link patients to aftercare and outpatient services as appropriate.
• Attend unit, facility meetings and case conferences as directed by Clinical Director.
• Attend mandatory trainings and pursue ongoing education in substance use and related topics.
• Coordinate with insurance providers for authorization when needed.
• Maintain campus safety and patient rights throughout duties.
