Job summary

Qualifications

Responsibilities

• Conducts physical therapy evaluation procedures according to professional and departmental guidelines.
• Completes evaluation and documentation within 24 hours of consult.
• Educates patient and caregiver regarding evaluation findings and management of care recommendations.
• Establishes measurable short- and long-term goals in collaboration with patient/caregiver.
• Implements comprehensive individual and group treatment plans and monitors progress.
• Communicates patient care and discharge needs to the team and arranges follow-up.

Education

Bachelor’s / Master’s or equivalent in Physical Therapy
