Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Cheyenne, WY area. You will provide primary patient care, including assessments, care planning, and education for patients receiving infusion therapy in home or alternate sites.

The role requires RN licensure in Wyoming, on-call availability, and the ability to drive to patients. Perks include mileage reimbursement, competitive wages, and a supportive team culture.

Qualifications

• Current RN license in Wyoming in good standing.
• RN degree, BSN preferred.
• Advanced infusion certifications preferred but not required.
• On-call rotation required including nights/weekends.
• On-site home infusion setting knowledge and safety awareness.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team.
• Educate patients/care partners on safely administering home infusion therapy.
• Assess home environment and identify safety interventions.
• Develop and evaluate care plans with the team.
• Coordinate scheduling and communicate changes to team members.
• Document patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure required POTs and documents are completed timely.
