Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Cranberry Township, PA area, performing assessments, care planning, and education in home or alternate sites. The role requires adapting to variable schedules and driving to patient homes as needed.

Responsibilities include direct patient care, coordination with care team, and documentation per policy. Multistate RN licensure and infusion experience are preferred, with mileage reimbursement and competitive

Formación

• Current RN license in good standing. Multistate RN to cover PA, WV, and OH.
• RN degree; BSN preferred.
• Advanced certification in CRNI/ONS (preferred but not required).
• Participation in an on-call rotation, nights and weekends as needed.

Responsabilidades

• Provide direct care for assigned patients, including infusion initiation and maintenance, and coordinate with pharmacists, prescribers, and care partners.
• Educate patients and care partners on home infusion therapy and safety.
• Assess home environment to ensure safe and appropriate care delivery.
• Develop and evaluate the care plan with the team for each encounter.
• Schedule management and coordination of care via the company platform.
