Job summary

Soleo Health seeks a Per Diem Infusion Nurse to support infusion patients in Wheeling, WV and surrounding areas. Must live within 25 minutes of Wheeling, WV. This role includes providing direct infusion nursing care in home settings, education, and coordination with the care team.

Responsibilities include patient assessment, care planning, on-call rotation, and ensuring compliance with policies. Multistate WV license preferred; BSN encouraged; CPR/CPNI knowledge beneficial.

Qualifications

• Current RN license in WV (multistate license acceptable)
• BSN preferred
• Infusion therapy experience preferred
• On-call rotation experience preferred
• CPR/ACLS knowledge a plus

Responsibilities

• Provide direct care for infusion patients in home/alternate sites
• Educate patients and care partners on home infusion therapy
• Assess home environment and identify safety interventions
• Develop and evaluate plan of care with the team
• Coordinate care and manage scheduling via approved platform
• Participate in nurse on-call rotation and document encounters
