Job summary

Soleo Health is hiring a Per Diem Infusion Nurse to support infusion patients in Cranberry Township, PA and surrounding areas. The role involves direct patient care, education, and coordination for home infusion therapies.

Responsibilities include evaluating patient needs, collaborating with pharmacists and prescribers, and ensuring timely documentation and regulatory compliance. Requires multistate RN license and willingness to cover PA, WV, and OH.

Qualifications

• Current RN license in good standing; multistate RN to cover PA, WV, OH.
• BSN preferred; advanced certification (CRNI/ONS/IgRN) desired but not required.

Responsibilities

• Provide direct care for assigned patients, including initiation and maintenance of communications with care team members.
• Educate patients and care partners to safely administer home infusion therapy.
• Assess home environment and identify interventions to ensure safe care at home.
• Develop and evaluate the plan of care with the team; monitor response to therapy.
• Coordinate patient care via approved company platform and manage schedule per policy.
