Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Dublin, OH and surrounding areas. The role involves direct patient care in home or alternate sites, with a focus on infusion therapy, assessments, education, and care planning.

Must live within 25 minutes of Dublin, OH and participate in an on-call rotation. Hourly compensation includes mileage reimbursement and an on-going commitment to Soleo’s protocols.

Qualifications

• Current RN license by Ohio Board of Nursing
• RN degree, BSN preferred
• Advanced certifications in infusion specialty preferred (CRNI/ONS/IgRN)
• Participation in on-call rotation including nights and weekends
• Administrative oversight responsibilities and resource management
• Emergency availability for non-standard hours as required
• Minimum 3 years nursing experience with infusion therapy
• ICU/Cardiac care experience preferred
• Proficient with Outlook and Microsoft Office
• Valid driver’s license with no significant restrictions
• Must live within 25 minutes of Dublin, OH

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team
• Educate patient/care partner for safe home infusion therapy
• Assess home environment for safety and appropriateness of prescribed care
• Develop and evaluate plan of care with the team
• Coordinate patient care via approved company platform
• Participate in nurse on-call rotation to ensure coverage
• Document all encounters and care activities promptly
• Maintain knowledge of Soleo therapies, protocols, and standards
• Ensure submission of POTs and clinical documents per policy
