Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Circleville, OH, and surrounding areas. You will provide direct infusion nursing care in patients' homes, educate families, and coordinate with the care team to ensure safe, effective treatment per policy.

Responsibilities include evaluating home environments, developing care plans, and documenting encounters. Requires Ohio RN licensure, 3+ years infusion experience, and ability to drive within 60 miles.

Qualifications

• Current RN license by Ohio Board of Nursing, in good standing.
• BSN degree preferred from an accredited institution.
• Minimum 3 years nursing experience with infusion therapy preferred.
• Must have a valid driver’s license with no significant restrictions on that license.
• Comfortable working in patients' homes around animals/cigarette smoke.

Responsibilities

• Provide direct patient care for infusion patients, including initiation and maintenance of communications with care team members.
• Educate patient and care partner to safely administer home infusion therapy and provide instructions.
• Assess home environment and identify interventions to ensure safe home care.
• Develop and evaluate the plan of care with other team members.
• Manage schedule and communicate scheduling changes per policy.
• Coordinate patient care via company platform and document encounters per policy.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Submit plans of treatment and clinical documents per state guidelines.
