Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Medina, OH and surrounding areas. You will provide direct patient care, perform assessments, care planning and education for home infusion therapy, and collaborate with pharmacists, prescribers and care partners to ensure safe, effective care.

Responsibilities include coordinating care via our platform, participating in the on-call rotation, and maintaining accurate documentation.

Qualifications

• Current RN license in good standing.
• Ohio RN license required.
• BSN degree preferred.
• Minimum 3 years nursing experience, infusion therapy preferred.
• On-call rotation, nights and weekends may be required.
• Ability to drive own vehicle for patient visits; mileage reimbursement available.
• Must live within 25 minutes of Medina, OH.
• Ability to work in home infusion settings and outpatient sites.

Responsibilities

• Provide direct patient care for assigned patients including initiation and maintenance of communications with care team members.
• Evaluate and educate patient/care partner to safely administer home infusion therapy.
• Assess home environment and identify needed safety interventions.
• Develop care plans in collaboration with the team and evaluate therapy response.
• Manage schedule and communicate any changes to team members.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation for continuous nursing support.
• Document patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POTs and required clinical documents.
