Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to perform In-Home Health Evaluations at patients' homes, bridging gaps in care. The role involves a 45-minute visit with history review, physical evaluation, and potential tests as per health plan orders.

Ideal candidates hold an active MD or DO license, may have multi-state licenses, and must be willing to travel locally within licensed states. Part-time schedule with hourly pay and benefits for part-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
