Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide In-Home Health Evaluations. You’ll visit patients in their homes, perform health assessments, review medications, and conduct diagnostics as ordered by health plans. The role supports flexible local travel and virtual visits as needed.

Eligible candidates hold MD/DO licenses in good standing and may have multi-state licenses. This part-time position focuses on delivering personalized, comprehensive health evaluations

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Perform in-home health evaluations and reviews of medications and medical history.
• Conduct physical examinations and necessary diagnostic procedures in the home setting.
• Coordinate with health plans to connect members with the right care.
