Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, visiting members in their homes to bridge gaps in care.

You will conduct 45‑minute visits, review medications and history, and perform a clinical evaluation using our iPad workflow. This part-time role requires local travel within licensed states and may include virtual visits as needed.

Qualifications

• Active, unrestricted medical license(s) in the states where you practice.
• Multi-state licenses are preferred and may be required for coverage.

Responsibilities

• Conduct 45-minute in-home health evaluations.
• Review medication histories and medical history; perform physical evaluation when appropriate.
• Use clinical workflow on an iPad; do not prescribe or order labs unless required by the plan.
