Signify Health, part of CVS Health, seeks a Per Diem Physician to deliver In-Home Health Evaluations, engaging with patients in the comfort of their homes to bridge gaps in care.

Visit duration is about 45 minutes, including review of medication history and physical assessment; tests may be added per plan. Travel within licensed states is required; virtual visits also occur as needed.

Formación

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsabilidades

• Bring your heart into every visit, joining a national network of clinicians.
• Visit millions of people nationwide to connect them with the right care.
• Collaborate with Signify Health as a clinician and fulfill role requirements.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits when needed.
• Provide diagnostic and preventive services as indicated (e.g., spirometry, bone density).
