Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

You’ll play a critical role in building trusted relationships to make people healthier by delivering flexible, comprehensive health evaluations at members’ homes. This part-time role uses an iPad workflow and requires clinical expertise with licensure in coverage areas.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations.
• Engage with patients to bridge gaps in care.
• Document findings and coordinate with health plans and PCPs.
• Travel locally within licensed states; conduct virtual visits as needed.
• Use iPad and clinical workflow; no prescribing or lab ordering required.
