Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations, engaging with patients in their own homes and bridging gaps in care. This role supports in-home assessments with a focus on safety, quality, and compassionate care.

The position offers a part-time schedule (12 hours weekly). Travel locally within licensed states is required, with potential virtual visits as needed and a base hourly pay of $105.00.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations in the patient’s home.
• Engage with patients to review medication and medical history, perform physical evaluation and order diagnostics as needed.
• Travel locally within licensed states as required, conduct virtual visits when appropriate.
