Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, engaging with patients in the comfort of their own homes.

You’ll visit members, review medication and medical history, perform a clinical evaluation, and coordinate with health plans. This role requires travel within licensed states and may include virtual visits as needed, with a competitive hourly base pay.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Build trusted relationships to connect patients with the right care.
