Responsibilities

• Provide In-Home Health Evaluations for CVS Signify Health.
• Engage with people in their homes to bridge gaps in care.
• Travel locally and within licensed states to conduct visits.
• Conduct virtual visits as needed; perform health evaluations.
• Utilize Signify Health services including diagnostic and preventive testing.
