Job summary

Signify Health, part of CVS Health®, seeks a Per Diem Clinician (NP or PA) to deliver in-home health evaluations directly at members' homes. You will connect with individuals, collect medical history, perform physical assessments, and provide diagnostic services per plan where applicable.

This role offers a part-time, per diem schedule with travel across licensed states, and includes malpractice insurance, supplies, and other perks.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification (NP or PA) is required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Work with Signify Health clinicians to deliver services as indicated.
• Travel locally within licensed states and conduct virtual visits when needed.
