Ennoble Care LLC is seeking a compassionate Hospice Social Worker for a per diem role in the Arlington, VA region. You will provide emotional support, counseling, and resources to patients and families, ensuring comprehensive hospice care.

The role requires an MSW with LMSW licensure, 1+ year post-master’s experience in healthcare, and ability to handle confidential information. Hospice experience is preferred.

Qualifications

• MSW degree from a graduate school accredited by the CSWE.
• LMSW license is required.
• 1+ year post-master’s social work experience in healthcare.
• Hospice experience preferred.
• Ability to maintain patient confidentiality.

Responsabilités

• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings and care plan development.
• Document initial evaluations, care plans, and daily notes.
• Monitor patient and family coping with dying and respond to changes.
• Coordinate visits with Case Managers and other staff.
