Qualifications

• One year of Social Work experience in a healthcare setting is mandatory.
• Preferred one year of hospice experience.
• Must possess a licensed driver’s license and an insured automobile.

Responsibilities

• Conduct psychosocial assessments within 5 days of admission.
• Participate in the development of the patient’s plan of care.
• Maintain and submit clinical records as required.
• Confirm weekly visits with the patients’ Case Managers.
• Participate in discharge planning as needed.
