Ennoble Care is seeking a compassionate Hospice Social Worker to join our Arlington, VA team on a per diem basis, with potential full-time benefits. The role provides emotional support, counseling, and resources to patients and families while ensuring comprehensive hospice care.

Responsibilities include psychosocial assessments, care plan development, and coordination with the IDG and Case Managers. A Master’s in Social Work and LMSW license are required, with hospice experience preferred.

Formación

• Must hold a VA State LMSW license.
• Master’s degree in social work from a CSWE-accredited program.
• Hospice experience is preferred.
• Ability to reliably handle confidential information.
• Must have a valid driver’s license and insured vehicle.

Responsabilidades

• Conduct psychosocial assessments within 5 days of admission to hospice.
• Participate in IDG meetings and develop patient care plans.
• Observe and report changes in emotional/social factors affecting care.
• Maintain and submit initial evaluations, care plans, and daily notes.
• Evaluate patient/family responses to social work interventions.
• Coordinate visits with Case Managers on a weekly basis.
