Job summary

Optum Geisinger Hospice-Lewistown is seeking a Social Worker to provide medical social services under physician and interdisciplinary group direction to address social and emotional factors affecting the patient’s health.

Responsibilities include helping patients and families understand personal, emotional, and environmental factors; contributing to a comprehensive Plan of Care; and teaching the team about community resources as needed.

Qualifications

• Master’s Degree in Social Work from CSWE-accredited program.
• Current driver’s license and access to a dependable vehicle or public transportation.

Responsibilities

• Assist patient and significant others, physician and health care team to understand social factors related to health status.
• Contribute to development of a comprehensive, integrated Plan of Care for patients daily.
• Instruct health care team members on community resources available to assist patients as needed.
