Job summary

Optum is seeking a Hospice-Medical Social Worker in Danville, Pennsylvania, to provide medical social services under the direction of a physician and the Interdisciplinary Group. Your role will assist patients in understanding significant emotional and social factors related to their health and aid in the development of coping strategies.

You will work collaboratively with healthcare teams to create comprehensive care plans and supervise bereavement services as necessary. The ideal candidate will possess a Master's Degree in Social Work and at least one year of healthcare experience.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• 1+ years of social work experience in a health care setting.

Responsibilities

• Assist patients to understand significant emotional and social factors related to health.
• Contribute to the development of the Plan of Care for patients.
• Instruct health care team on community resources available.
• Function as Bereavement Coordinator and supervise bereavement services.
