Job summary

UnitedHealth Group's Idaho Home Health and Hospice is seeking a Social Worker - Hospice Per Diem to provide medical social services under a physician and the Interdisciplinary Group, helping patients and families understand social and emotional factors affecting health and coping.

The role supports care planning, bereavement services when needed, and coordination with the health care team. Hourly pay ranges from $30.35 to $45.50, with comprehensive benefits and per diem scheduling.

Qualifications

• Master's Degree from a CSWE-accredited program.
• 1+ years of social work experience in a health care setting.
• Current Driver's License and access to dependable transportation.

Responsibilities

• Assist patients and families to understand significant social and emotional factors related to health status.
• Contribute to the development of a comprehensive Plan of Care for patients.
• Instruct care team on community resources available to assist patients as needed.
• Coordinate bereavement services and establish a care plan addressing bereavement needs.
