Job summary

Vitality Hospice is seeking a Per Diem Registered Nurse to join our patient‑centered team. You will perform admissions, assessments, and ongoing evaluations, developing individualized care plans with the interdisciplinary team.

You will travel within assigned service areas, supporting home health aides, social workers, and chaplains, while maintaining thorough documentation and promoting quality care in the community.

Qualifications

• Active NJ RN license.
• Minimum 1 year of RN experience.
• Willingness to travel within assigned service areas.
• Valid driver’s license, reliable transportation and active auto insurance.

Responsibilities

• Provide professional nursing care, including admissions, initial assessments and ongoing evaluations.
• Develop, implement, and update individualized plans of care based on patient needs.
• Collaborate closely with hospice physician and Interdisciplinary Team for interventions.
• Oversee and support home health aides, social workers and chaplains.
• Educate patients and caregivers to promote safety, comfort and understanding.
• Maintain high standards of documentation, compliance and clinical excellence.
• Contribute to continuous improvement to elevate quality of care.
