Job summary

Pathways Healthcare is seeking an experienced registered nurse for per diem home health in the Boston, MA area. You will assess patients, plan and implement individualized home care, and collaborate with physicians and interdisciplinary teams to ensure high-quality outcomes.

Responsibilities include administering meds, nursing procedures, discharge planning, and supervising home health aides while maintaining compassionate, patient-centered care in patients' homes.

Qualifications

• Active Nursing license in the state of position.
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of care plans.
