Job summary

Pathways Healthcare in Westwood, MA, is seeking a per diem Registered Nurse to deliver in-home care across Mansfield and nearby areas. You will initiate nursing procedures, assess patients, and develop care plans in collaboration with physicians and the care team.

Qualifications

• Active RN license in the state.
• 2+ years nursing experience, geriatrics preferred.
• Nursing degree from accredited program.
• Excellent bedside manners and communication.
• Knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver's license and insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
