Job summary

Dierksen Hospice in Hot Springs, AR is seeking a Per Diem Hospice RN Case Manager to join our compassionate nursing team delivering end-of-life care. You’ll coordinate services, support patients and families, and work within an interdisciplinary team to provide The Best Care Possible.

The RN must hold an unencumbered RN license, have at least 1 year in medical-surgical/acute care, CPR if required, EMR proficiency, and a valid driver’s license with reliable transportation.

Qualifications

• Current unencumbered RN license required.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification if required by state.
• Proficiency in computers/EMR; valid driver's license and reliable transportation.

Responsibilities

• Empower Through Engagement: respond to clinical referrals with care and expertise.
• Collaborative Care Champion: assess eligibility and suitability for hospice services with Clinical Director.
• Nurturing Direct Care: deliver hands-on hospice nursing care and ensure patient comfort.
• Nursing with Heart: apply nursing skills to ease pain and provide solace.
• Educator and Advocate: educate patients and families to create personalized care plans.
• Call of Compassion: participate in on-call duties to be there when it matters most.
