Job summary

Dierksen Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager to join our care team serving the Jonesboro community. You will deliver compassionate end-of-life nursing care, coordinate services, and guide patients and families through their hospice journey.

The RN will collaborate with our interdisciplinary team, provide education, and participate in on-call coverage. This role requires an active RN license, at least 1 year of RN experience, EMR proficiency, and reliable

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services for patients and families.
• Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
• As an RN Case Manager, provide comfort and quality of life through direct care coordination.
• Apply nursing skills to ease pain and support patients and families.
• Educate patients and families to empower participation in personalized care plans.
• Participate in on-call duties as part of the team.
