Job summary
Dierksen Hospice is seeking a dedicated RN Case Manager to deliver hands-on hospice nursing care within the Texarkana community. You will coordinate services and guide patients and families through the hospice journey, working closely with our interdisciplinary team to ensure compassionate, high-quality care.
Qualified candidates will hold an active RN license, have at least 1 year in medical-surgical/acute care, and possess EMR proficiency.
Qualifications
- Current unencumbered RN license.
- Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
- Computer proficiency and EMR background.
- Current CPR certification if required by state.
- Valid driver's license and dependable transportation.
Responsibilities
- Deliver hands-on hospice nursing care and coordinate services.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Provide comfort and quality of life improvements for patients.
- Apply nursing skills to ease pain and support families.
- Educate patients and families to participate in care plans.
- Share call duties and respond to after-hours needs.
