Job summary

Elite Hospice, part of the Optum family, is hiring a Per Diem Registered Nurse Case Manager to oversee hospice patient care at home. You will conduct initial evaluations, develop plans of care, and document care while maintaining quality and productivity.

The role emphasizes compassionate, patient-centered guidance within the home setting. Requirements include a current RN license, 1+ year clinical experience, a valid driver's license and access to reliable transportation.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR Certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family needs and reassess every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG to deliver daily care.
