Job summary

Alpha Omega Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our nursing team delivering hands-on hospice care throughout the community. Schedule: Per Diem, Monday - Friday, 8 am - 4:30 pm, On-Call After Hours. Territory: Ellis and Dallas County.

You will coordinate services, guide patients and families, and ensure compassionate, high-quality end-of-life care within our interdisciplinary team. The pay range is $36.00 - $42.00 per hour with compensation discussed at interview.

Qualifications

Current unencumbered Registered Nurse license.
Minimum one year RN experience in a medical-surgical/acute care setting; hospice experience a plus.
Proficient with computers and EMR systems.
CPR certification if required by state.
Valid driver’s license and access to dependable transportation.

Responsibilities

Respond to clinical referrals with care and expertise while addressing inquiries and requests.
Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
Provide direct nursing care to ensure patient comfort and quality of life.
Deliver nursing with compassion and apply proven nursing principles.
Educate patients and families to create personalized care plans aligning with goals.
Participate in call duties to be there when it matters most.
