Job summary

Alpha Omega Hospice is seeking a Full-time Hospice RN Case Manager to join our compassionate nursing team delivering end-of-life care across Carrollton and surrounding areas.

The role involves hands-on clinical care, care coordination, and guiding patients and families through the hospice journey, with a Monday–Friday 8:00 am to 4:30 pm schedule.

Qualifications

• Active RN license required.
• Minimum 1 year in medical-surgical/acute care; hospice experience a plus.
• Proficient with electronic medical records (EMR).
• CPR certification if required by state.
• Valid drivers license and dependable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• As an RN Case Manager, provide direct nursing care to ensure patient comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques to ease pain and bring solace.
• Educate patients and families to participate in personalized care plans that align with goals.
• Share call duties as part of the team to be there when it matters most.
