Job summary

Hospice of the West is seeking a Per Diem Hospice RN Case Manager to deliver hands-on nursing care and coordinate services for patients and families in the Casa Grande community. You will work with an interdisciplinary team to guide the hospice journey with compassion and clinical excellence.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp helpful.
• Must be computer proficient with typing and EMR proficiency.
• CPR certification current if state requires.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise, addressing inquiries compassionately.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct nursing care to maintain patient comfort and quality of life.
• Deliver nursing care rooted in established principles and compassionate practice.
• Educate patients and families to participate in personalized care plans.
• Share call duties as part of the team.
