Job summary

Hospice of the Valley in the Phoenix area is seeking an experienced RN Case Manager to provide and coordinate hospice care for home care patients in the Maricopa and Casa Grande territory. Responsibilities include guiding the interdisciplinary team, educating patients and families, and ensuring symptom control in patients' homes.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license.
• Minimum 1 year of medical, surgical or critical care experience.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Ability to manage time and attention to detail.

Responsibilities

• Develops and maintains therapeutic relationships with patients and families.
• Plans and coordinates hospice home care for patients.
• Provides pain/symptom assessment and management.
• Facilitates transitions between care environments.
• Educates patients and families about disease processes and care options.
• Collaborates with interdisciplinary hospice team to ensure comprehensive care.
• Documents care timely and accurately.
• Utilizes resources effectively and efficiently.
• Adheres to scope of practice and organizational standards.
• Maintains professional skills and conduct.
