Job summary

UnitedHealth Group in North Atlanta seeks an experienced RN Case Manager to oversee the Plan of Care for hospice patients, collaborating with the IDG to implement strategies addressing physical, psychosocial, emotional, and spiritual well-being.

The role requires an active RN license, 1+ year of clinical experience, and a valid driver’s license with reliable transportation. Full-time and PRN options are available, with competitive compensation and benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family needs every 15 days at minimum.
• Initiate and coordinate the plan of care with the IDG and attending physician.
• Document goals, interventions, and patient/family responses to hospice care.
• Educate and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
