Job summary

Atlantic Health's Atlantic Visiting Nurse is seeking a Per Diem Hospice Registered Nurse to provide compassionate inpatient hospice care at Morristown Medical Center and Overlook Medical Center. The role offers weekend and on-call opportunities with autonomy in patient-centered care.

Responsibilities include comprehensive nursing assessments, care plan development, symptom management, education for patients and families, EMR documentation, and collaboration with the interdisciplinary team to

Qualifications

• Active New Jersey RN license required.
• ASN required; BSN preferred.
• Current BLS certification and clean driving record.

Responsibilities

• Perform comprehensive nursing assessments and hospice appropriateness.
• Develop and evaluate individualized patient/family care plans.
• Coordinating care with physicians and inter-disciplinary team.
• Educate patients and families on hospice care and meds.
• Maintain EMR documentation and regulatory compliance.
• Participate in quality improvement and staff mentoring.
