Job summary

Atlantic Health System/Atlantic Visiting Nurse (AVN) is seeking an On-Call Registered Nurse in Morristown, NJ to provide after-hours patient care in coordination with the IDT. You will respond to triage requests, manage pain and symptoms, and support hospice admissions while ensuring quality and regulatory compliance.

You will collaborate with physicians, IDT members, and families to adjust plans of care, document care, and educate patients and caregivers on hospice philosophy and disease

Responsibilities

• Provides after hour on call coverage as scheduled between 4pm and 8am.
• Responds to triage calls and provides patient visits on request of triage nurse/Care XM RN.
• Responds to medical or emotional crisis and/or death as requested by triage nurse/Care XM RN.
• Delivers skilled nursing care focusing on pain control and symptom management.
• Conducts hospice admissions as needed.
• Coordinates care with attending physician and medical directors for orders.
• Assists RNCMs with after-hours visits to meet patient needs.
• Maintains patient privacy, rights and confidentiality.
