Qualifications

• Current New Jersey Registered Nurse (RN) License.
• Minimum one year of nursing experience in Acute, Subacute, Home Health, or Hospice.
• Experience using an Electronic Medical Record (EMR).
• Strong clinical assessment and critical thinking skills.
• Excellent verbal and written communication skills.
• Ability to work independently while managing a patient caseload.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans.
• Serve as primary Case Manager for assigned patient caseload and coordinate with the IDT.
• Educate patients and families on disease progression, medications, and hospice philosophy.
• Complete timely clinical documentation in the EMR and adhere to agency standards.
• Demonstrate knowledge of hospice regulations and Medicare guidelines.
• Maintain safe medication administration and infection prevention practices.
• Participate in Performance Improvement initiatives and mentor new staff.
