Job summary

Horizon Hospice Care is seeking a PRN Registered Nurse to provide comprehensive in-home or facility-based hospice care. The RN will perform assessments, develop and implement care plans, and collaborate with physicians and the interdisciplinary team.

The ideal candidate holds an active RN license, is CPR certified, and has hospice experience. A driver’s license is required to visit patients, and ongoing education is encouraged.

Qualifications

• RN with current licensure to practice in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver’s license.
• At least one year of Hospice experience (preferred).

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care using all elements of the nursing process.
• Develops and revises plans of care with input from patient and family.
• Administers medications and treatments as prescribed.
• Counsels patient and family in nursing care needs.
• Teaches self-care techniques to patient and family/caregiver.
• Acts as Case Manager when assigned by Clinical Supervisor.
• Maintains accurate clinical notes and reports changes in patient status.”
• Coordinates care with physician and interdisciplinary team.
