Job summary

Chapters Health System is seeking a Per Diem RN to assess patient and family needs, coordinate Plan of Care with the Interdisciplinary Group (IDG), and provide palliative and supportive care.

Requires current RN license in the state, BLS certification, and at least 1 year of nursing experience (hospice/hospital preferred). EMR/EHR experience and a valid driver's license are required; English communication skills are essential. Flexible schedule offered.

Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience; hospice or hospital experience preferred.
• Experience working with an EMR/EHR system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet patient care needs.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patient, family, caregivers on disease, care, and safety practices.
• Reports changes in patient condition to IDG or other health professionals.
• Participates with the IDG to evaluate hospice referrals/admissions for level of care.
