Job summary

Constellation Health in Brockton, MA is seeking a Nurse Practitioner with Hospice/Palliative Care focus to provide PRN/per diem hospice services. Under physician supervision, you will assess patients, develop care plans, prescribe meds, and coordinate with families and the hospice team.

Applicants must have an accredited NP program, current MA nursing license, DEA, CPR, and hospice/home care experience. Strong communication, clinical assessment, and ability to work independently are essential.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license.
• Current nursing licensure in state and CPR certification.
• Licensure, education, training, and qualifications required by state rules/regulations.
• Accreditation from an approved certifying body for advanced practice nursing (required).
• Certification in a specialist area preferred (e.g., CHPN).
• Excellent observation, verbal and written communication, problem solving, and nursing skills per competency checklist.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical exams and document findings.
• Develop individualized plans of care and coordinate hospice services.
• Complete hospice face-to-face visits and assist with eligibility determinations.
• Collaborate with hospice interdisciplinary team to ensure symptom management.
• Communicate with patients, families, and healthcare practitioners to coordinate care.
• Educate patients/families on preventative care and end-of-life issues.
• Maintain clinical notes and documentation in the medical record.
• Attend hospice interdisciplinary group meetings and participate in quality initiatives.
