Job summary

Constellation Health is seeking a dynamic Nurse Practitioner with Hospice/Palliative focus to work on a PRN/Per Diem basis in Fairfax County, VA. The NP provides comprehensive hospice services under physician supervision, evaluating health status, planning care and coordinating with families and the interdisciplinary team.

The ideal candidate has hospice/palliative experience, an active NP license, DEA, CPR, and a willingness to travel to patients' homes.

Qualifications

Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
Current DEA license.
Current nursing licensure in state and CPR certification.
Licensure requirements by state and CPR as applicable.
Prior home care/hospice experience preferred.
Certification in Hospice and Palliative Nursing (CHPN) or related specialties preferred.
Active driver's license with reliable vehicle.
Strong observation, communication and analytical skills.
Ability to stand/walk for extended periods and assist with patient transfers.

Responsibilities

Provide medical care and symptom management for terminal illness under physician supervision.
Perform history and physical examinations and document findings.
Develop individualized care plans and coordinate hospice services.
Complete hospice face-to-face visits for eligibility assessments.
Provide recommendations on patient eligibility for hospice certification/recertification.
Coordinate with hospice interdisciplinary team and other providers.
Ensure symptom control and documentation of care plans.
Develop protocols for treatment and discuss options with supervising physician.
Communicate plan of care with patients, families and team.
