Job summary

Constellation Health is seeking a Nurse Practitioner focused on Hospice/Palliative Care for a PRN/Per Diem role in Rockland, Maine. You will provide comprehensive hospice services under physician supervision, conducting exams, developing care plans, and coordinating with the interdisciplinary team to meet patient needs at home.

Qualifications

• Graduation from an accredited nursing school and NP program.
• Current DEA license and CPR certification.
• State NP licensure and hospice/home care experience preferred.
• Certification in Hospice and Palliative Nursing (CHPN) or similar preferred.

Responsibilities

• Provide hospice medical care under physician supervision.
• Perform history, exam, and order/interpret tests, document findings.
• Develop individualized plans of care and prescribe treatments.
• Complete hospice face-to-face visits to assess eligibility.
• Coordinate with interdisciplinary team and educate patients/families.
